For paper prescriptions from your physician:
Submit your request online via our order form below then mail us your paper prescription along with the order number which you will be given after you fill out the order form.
No paper prescriptions from your physician:
Submit your request online by filling out the order form. Our office will get in touch with your healthcare provider to file a prescription request which will be mailed directly to you.
